New Dental Patients
What to Expect at Your First Visit
There are more dental procedures today than ever before to provide comprehensive solutions. We are committed to helping you understand your options and access affordable treatment. Your first visit will be a consultation with Dr. Lisk to determine the state of your oral health. This will help us verify if you need a routine dental cleaning, a deep cleaning to correct early stage gum disease, or if we need additional time to address restorative or aesthetic concerns.
New Patient Forms
We understand it can be difficult to take time out of your busy day for appointments and want your visit to our Raleigh dentistry practice to be stress-free. If you would like to streamline the check-in process, we suggest downloading and filling out our new patient forms prior to your visit.
Confidential Patient Information
Acknowledgement of Receipt Of Notice of Privacy Practices
Authorization for Release of Information
This is an important step and provides Dr. Lisk with insight into your overall wellbeing. If you prefer, you can fill out your new patient paperwork when you arrive for your visit. However, we do request that you come in about 15 minutes prior to your appointment so you have ample time to provide your dental and health history. This helps us to keep your appointment, and the appointments of others, on track.
